How to report a petroleum incident
All incidents (including near misses) at onshore petroleum operations (including Barrow Island) and petroleum pipelines must be reported to:
Department of Mines and Petroleum:

Follow-up written reports where required to:- reports.upstream@dmp.wa.gov.au
