This form should be completed by the operator of a dangerous goods site that is not a classified major hazard facility if the operator intends to store or handle more than the critical quantity of Schedule 1 substances.

Notification to the Chief Officer

A notification should be completed by the operator of a dangerous goods site that is not a classified major hazard facility if:

  • the operator intends to store or handle more than the critical quantity of Schedule 1 substances on site

or

  • more than the critical quantity of Schedule 1 substances is present on site and the operator intends to implement a notifiable change.

Important note: A notifiable change is defined in regulation 15(1) of the Dangerous Goods Safety (Major Hazard Facilities) Regulations 2007. It includes a significant change to plant, processes or substances, or the layout of the site.

The notification will be assessed to determine whether the Chief Officer will classify the site as a major hazard facility (MHF).

Lodging the notification

The completed and signed notification form and attachments (if any) should be mailed to:
