Applicant and Company - (External)
This page provides information on how to complete the Applicant and Company information for Health Management Plans.
Mining
The company and combined operation / site operation is listed, by default.
Click 'Add'.
Where the user access (security) to more than one combined operation / site operation, The Combined Operation and corresponding site operations (SG) will be available in the dropdown. Only site ops (SGs) under the same combined operation can be added.
Click 'Add'; or 'Add All' to add the site operations.
To clear, click 'Clear'.
Exploration
Enter the operation name.
Update / Delete an incorrectly added site operation
Click the 'Edit' pencil icon. This displays the site operation in the field above.
Note: 'Edit' allows you to replace an existing site with another.
Click 'Update'.
To delete, click the 'delete' icon to remove a site previously added.
On the prompt, click 'Yes'.
Contact Persons
Only users with relevant security roles will be available in the dropdown.
Click to select the name.
Enter the reason for the contact e.g. "general enquiries regarding ..." or; "Contact for "site operation xxx for enquiries regarding ..."
Click 'Add'.
To add more than one name, select and click 'Add' after each selected name.
Form Completed By
The name is based on the user's log in, by default.

Revision Submissions
The site operation(s) will be listed by default.
To add another site under the same Combined Operation select from the dropdown and click 'Add' / 'Add All' .
The 'Edit' / 'Delete' icons are available to update / remove sites.
Annual Review
The Company, Combined Operation and Site Operation (s) will be listed by default.
