Application Type

So that your application may be processed, the type of application being submitted must be clearly identified, e.g.:
  • Application for a new licence   (N)

If you'd like to have the licence to be aligned with your common expiry date, check the "Yes" option and please provide your existing licence expiry date in the field provided.

This eForm is currently not set up to accommodate amendments to dangerous goods storage on an existing Dangerous Goods Site Licence. Please use the hard copy application form which can be found on the Resources Safety website.
