Type Date and Time under MSIA 1994

Enter the date of the incident (format dd/mm/yyyy format; or select from the date picker.
Select Known and enter time in the 24-hour format (hh:mm) or am/pm format (h:mm am/pm)
Select Unknown and enter the reason in the free text field.
