Transitional arrangements may be in place for the compliance requirements on this page.

The information below has been left for historical compliance reference purposes
Consultation
A PIN can only be issued after consultation between the elected safety and health representative and the supervisor of the relevant activity has failed to resolve the issue within a reasonable time.
Consultation can provide an opportunity for the alleged breach to be addressed without the need to issue a PIN, and supports a positive safety culture.
PIN forms
Each form contains detailed instructions covering the duties of each person involved.
To issue a PIN the elected safety and health representative must:
- have completed the required safety and health representative training
- be elected for the relevant area or group where the contravention occurred.

Request for inspection
The person to whom a provisional improvement notice is given may, within 7 days, request a petroleum safety inspector to review the subject matter of the notice. The PIN is suspended for the review period.
The inspector can cancel, uphold or modify the notice. If upheld or modified the PIN becomes an improvement notice with the rights to review and appeal.
To request a PIN review, please telephone and ask to speak to a petroleum safety inspector on +61 8 9222 3333.
Cessation of notice
A PIN ceases to have effect when it is either cancelled by the SHR or an inspector, or the responsible person takes action specified in the notice, or takes action to prevent the contravention described in the PIN.
