This template is regarding the notification of proposed exploration activities. The form is read-only. Please save a copy to your computer, prior to completing and submission.

Who is responsible for the notification?

The exploration manager is required to notify the senior inspector of mines, for the region where exploration activities take place, about the:

  • location
  • scope
  • nature
  • general safety
  • date of activity

of work to be undertaken.

Lodging the notification

The completed form and details of the schedule of works (if applicable) can be emailed using the ‘submit by email’ button at the bottom of the form. This will ensure the notification is forwarded to the correct inspectorate and create the subject heading the department requires.
