Who is responsible for reporting?
The occurrence report form is used to report accidents and dangerous occurrences at or near facilities in offshore coastal waters (up to 3 nautical mile limit).
A representative of the operator of the petroleum operation or geothermal energy operation is responsible for submission of the report.
Lodging an occurrence report
For electronic submission of the form (and any attachments where applicable) either:
- use the ‘Submit by email’ button on the form
or
- email to petreps@dmirs.wa.gov.au
