Who is responsible for reporting?
The occurrence report form is used to report the occurrences and injuries (both lost time and alternative duties) that occurred on the site during the month.
A representative of the operator of the petroleum operation or geothermal energy operation is responsible for submission of the report.
The report must be submitted within 3 days of the incident.
Lodging an occurrence report
For electronic submission of the form (and any attachments where applicable) either:
- use the ‘Submit by email’ button on the form
or
- email to petreps@dmirs.wa.gov.au
