Who is responsible for reporting?

If an accident or occurrence (commonly referred to as a notifiable incident for reporting purposes) happens on:

  • a mine site
    • the manager of the mine site should submit the report.
    • this includes exploration activities on a mine site
    • the report should be included in the sites figures.
  • an exploration lease
    • the exploration manager should submit the report.

General requirements

For reportable accidents, incidents and injuries, there are several avenues of reporting.

Telephoning WorkSafe via 1800 678 198 if serious or potentially serious.

  • Reporting online through the Safety Regulation System (SRS)
    • notifiable incident report (NIRF) for an occurrence
    • mining injury report
    • submission of the monthly status report (whether or not there has been a reportable accident for the month).

Hydraulic or paste fill incidents or barricade failures require additional information after the initial occurrence report. Information gathered during the incident investigation must be submitted to the district inspector within 2 weeks of the initial report.
