Notification of change of details
The department’s Chief Dangerous Goods Officer must be informed within 14 days of any material changes to the details on a licence, registration and Dangerous Goods Security Card, including:
- change of holder name (individual, company, partnership or trust)
- change of business trading name
- change of residential, business or postal address
- correction to date of birth.
The change of details form cannot be used for:
- circumstances where a licence is no longer required, or cannot continue to exist (e.g. death of the holder, dissolution of a partnership or company) - please contact Resources Safety
- amendment of specific authorisations granted by licences, such as product details or driver licence classes - please use the application form appropriate to the licence
- transfer of a licence - please use the relevant licence transfer application form.
The transfer of a personal explosives or Security Sensitive Ammonium Nitrate (SSAN) licence is not permitted.
The transfer of an explosives trading licence or Security Sensitive Ammonium Nitrate (SSAN) licence is permitted by means of a licence amendment.
Change to driving record
If a driver’s licence is cancelled, the licence holder must inform Support Services by email cso@dmirs.wa.gov.au.
Amendment, transfer or upgrade of licence or registration details
Transferable licences or registrations
An individual, partnership, body corporate or a trust may hold a licence or registration. The type of applicant determines which form is required to amend or transfer a licence or registration:
- individual applicant wishing to change name or address - use a change of details form
If you are a body corporate, partnership or trust changing the name and you have retained your ACN you can use the change of details form.
- body corporate, partnership or trust wishing to transfer to a new operator or amend a licence or registration details (including transfers or changes to storage inventory on licence or registration) - submit an application form along with the relevant supporting documentation.
For the following licences, individuals need to notify changes of name or address using the Change of details form only.
Body corporates, partnerships and trusts need to transfer to a new operator or amend their licence.
Non-transferable licences
The licences listed below are held by individuals and are non-transferable.
Changes to personal details can be dealt with by using the change of details form.
If the licence needs upgrading to reflect a change in competency, an application form must be submitted with the new competency information (i.e. certificate, licence).
For the following licences, individuals need to notify changes of name or address using the Change of details form only.
Body corporates, partnerships and trusts need to transfer to a new operator or amend their licence.

Application to change details
If you hold a dangerous good driver licence, and have access to SRS, you can log on to SRS to amend any of your personal details. Any change will flow through to other licences you hold.
If you have an existing SRS Ex account, Log on to SRS
Enter your Ex-Account number and password and follow online instructions to complete your application
If you also hold a dangerous goods security card (DGSC) and you amend your name in a way that requires a new signature, following your submission, you will be requested to submit a new signature for your DGSC.
Upgrading classes
If you do not have an existing SRS Ex account visit the upgrade SRS new user page. Follow the online instructions to complete your application.
Changed circumstances
If you do not have an existing SRS Ex account visit the changed circumstance SRS new user page. Follow the online instructions to complete your application.
Complying with licence conditions
