Information for agents

Acting on behalf of individual

If requesting information on behalf of an individual involved in an accident or incident, you are required to provide an authority signed by the individual, allowing the department to release the information to you as the agent.

Acting on behalf of organisation

If requesting information on behalf of a business or company, please provide an authority from the business or company (on letterhead or with company email address), authorising the department to release the information to you as the agent. Although not required, this will assist with processing of the freedom of information request.

Application fee

If you as an applicant are requesting:

  • personal information or amendment of personal information, there is no fee
  • non-personal information, the fee is $30.00.

A processing charge may also apply to non-personal applications. If the processing charge is more than $25.00, an estimate of charges will be supplied. A 30-day period will be given for the applicant to advise if the request for access should continue.
